COLLATERAL SYSTEM 11 / 12
Order & intake forms
Landscape order forms and service intake forms that print right the first time.
Forms are the hardest print format to fake: every box has to be where a pen expects it. These are rendered and measured before they ship.
THE OUTPUT / GENERATED FOR APEX INSTRUMENTS
What it makes.
An 11 x 8.5 order form, a two-page intake form, and a cover page.

Apex Instruments, Northlight Group, and every name, price, and contact in these files are fictional demo data. The real versions of this system run inside client systems and are not shown.
THE MECHANISM / SHOW THE SHAPE, NOT THE SOURCE
How it is made.
- 01Ask
Name the partner and the form type.
- 02Recipe
The form skill fixes the grid: header fields, product grid, add-ons, delivery, notes, authorization, office-use strip.
- 03Assemble
Option lists are generated from the product records so a discontinued option cannot appear.
- 04Check
Rendered at print size, checked for overflow and for black-and-white legibility, since forms get photocopied.
- 05Ship
Landscape and portrait PDFs plus a cover page that says when to use which.
WHAT IT READS
- Product and option lists
- Service add-ons
- Submission routes
- Partner record for co-branding
WHAT IT CHECKS BEFORE IT SHIPS
- Every option traces to the product list
- Legible in black and white
- Signature and date lines present
- No overflow at 11 x 8.5
Counts only. Client and partner names, source files, and internal wording stay private. Every example on this page was regenerated for the fictional company rather than de-branded from client work.
